What You’ll Do

As a Home Health LPN, you’ll provide hands-on, compassionate care while working closely with an interdisciplinary team:

Deliver direct patient care in accordance with the state Nurse Practice Act
Carry out care plans developed by the Registered Nurse
Educate patients and families on self-care and treatment plans
Document care accurately and timely
Coordinate with RN and care team to ensure patients receive comprehensive support
Utilize proper techniques for equipment, supplies, and infection control
Support patients’ physical, emotional, and educational needs in the home

What You’ll Need

Active LPN/LVN license in the state
Graduate of an accredited practical/vocational nursing program
3+ years of nursing experience (Home Health, Med-Surg, or Community Health preferred)
Current CPR certification
Reliable transportation, valid driver’s license, and auto insurance
