About the Role
As a THL Care Coordinor, you will provide therapeutic interventions, case coordinion, and resource linkage to clients and other involved parties (i.e., family, DCS, Court personnel, non-relive supports, etc.) to support identified changes within the individual or family. This role focuses on delivering client-centered services and collaboring with stakeholders to improve outcomes for children, adolescents, and families.

· Provide therapeutic interventions to children and adolescents in a community-based trement environment
· Coordine care and services for assigned clients
· Link clients and families to approprie community resources
· Collabore with family members, DCS, court personnel, and non-relive supports
· Support identified behavioral and emotional goals for clients
· Maintain accure and timely documention in accordance with organizional and regulory standards
· Participe in team meetings and case reviews as required
· Ensure services are delivered in a competent, ethical, and client-centered manner

Required Qualificions
· Bachelor’s degree in a Human Service discipline from an accredited four-year college or university
· Minimum of 1 year of experience working with children and adolescents in a therapeutic, community-based trement environment
